Comments:
A Subsidy Health and Safety Inspection was conducted on October 2, 2018 review supplemental health and safety requirements. Licensing inspector arrived at 7:23am and departed at 9:00 am to conduct a Subsidy Health and Safety Inspection. There were 18 children present at arrival to the facility with two staff members. The children were observed arriving to the program, playing with various tabletop activities to include blocks, coloring, puppets and listening to music. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack would consist of yogurt and bananas. Three medications were reviewed. Five children's records were reviewed during the inspection. The exit interview was conducted with the Site Director. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, in two of five children's records reviewed, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The records for Child #1 and Child #4 indicated that the physicians diagnosed the children with food allergies and the center had not obtained a written allergy care plan for the children.

Plan of Correction: Site Director will talk to the parents and get the form and have them to fill it out.
