Today an unannounced monitoring inspection was conducted between the hours of 3:25pm and 5:40pm. I observed 64 children directly supervised by 4 staff. Ratios and supervision were in compliance. Children were observed playing on the playground, doing homework, doing crafts, participating in group play, and eating snack. Today's snack consisted on CapriSun-Fruit Punch 100% juice, Baked Cheddar Goldfish. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Required staff training documents were on-site. Standard #: 22VAC40-185-510-A Description: Based on review of medication and provider interview, it was determined that a child had a medications on site with an authorization form not signed by the parent. Evidence: Child A has an emergency prescription medications on site. The medication authorization form has been signed by the doctor, but it has not been signed by the parent. Plan of Correction: We will request new forms to be completed by the parent and the physician.
Standard #: 22VAC40-185-510-C Description: Based on medication review, the center did not have a current written medication authorization forms for long-term prescription medication. Evidence: Child B's records contain three long-term prescription medication authorization forms that expired on 8/12/2018. Plan of Correction: We will request an updated authorization form.
Standard #: 22VAC40-185-510-G Description: Based on review of medications, prescription medication was not maintained in the original, labeled container. Evidence: Two prescription medications for child B were not maintained in the original, labeled container. Plan of Correction: We will request the original containers.
