Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on October 2, 2018 from approximately 4:25 pm until 6:10 pm. There were 35 children in care and 4 staff present with children. Additional children arrived during the inspection. the toal enrollment for the program is 72 for the afternoon program and 74 for the morning program. The record review sample consisted of 6 children's records and the on site required documentation for 4 staff records. Children were observed eating snack consisting of chicken nuggets, french fries, fruit cup and milk. Children were also observed playing in centers to include blocks, dolls, board games and cars. Description: Based on documentation review and staff interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on October 2, 2018, the record for child #5 has documentation of a diagnosed food allergy (milk) and there is not a written allergy care plan. Center staff confirmed there was not a plan available for review.

Plan of Correction: The center supervisor will ensure there is an action plan for all children with diagnosed food allergies.
