Description: Based on review of ten children's records and discussion with staff on October 3, 2018, the program failed to ensure documentation of additional immunizations was obtained once every six months for two children under the age of two years. Evidence: 1. Review of records for child B and child J, on this date, determined that documentation of additional immunizations was not obtained once every six months. The most recent immunizations on file for twenty-three month-old child B were dated May 2017 and the most recent immunizations on file for one year-old child J were dated November 21, 2017 (signed January 22, 2018). 2. Staff #1 and staff #2 confirmed the ages of the children and immunizations on file.

Plan of Correction: The children's parents will be contacted and updated immunization records will be obtained.
