63.2 Facilities and Programs..

Technical Assistance:
Discussed with center management implementing a system to maintain ratios, overseeing the arrivals and departure of children when time for transitions.

Comments:
An unannounced monitoring inspection was conducted from 7:20am to 8:47am. There were 55 children in care of 3 staff members. The children were observed engaged in various activities such as arts and crafts, playing with dinosaurs, and playing with board games. The children were also observed transitioning to school after morning breakfast. Medications and emergency drill records were reviewed. Areas of non-compliance are listed in the Violation Notice. Standard #: 22VAC40-185-240-D-5
Description: Based on record review, staff members on duty did not obtain within the last three years instruction in performing the daily health observation of children. Evidence: Staff # 1's daily health observation training was done in December 2013 and expired in 2016. There was no one on staff with the daily health observation training.

Plan of Correction: I will make sure that I get that training.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and staff interview, one staff to 18 children ratio was not maintained at the center. Evidence: 1. There were a total of 55 children present with 3 staff members. 2. The age of the youngest child was 5 years. 3. Staff # 1 stated that one of their staff members was absent, making it hard for them to maintain ratio.

Plan of Correction: We need more staff and come up with a plan to divide the groups and maintain ratios.

Standard #: 22VAC40-185-510-A
Description: Based on observation, parental permission was not obtained by the center to administer prescription medication. Evidence: There was emergency medication for Child # 1. Center did not obtain written parental permission from the parents.

Plan of Correction: We will look for the form and if not found we will get a new form.
