Technical Assistance:
Had a discussion with center director regarding the additional online training that is required for MAT trained staff in order to administer a particular emergency medication. Not all MAT trained staff need to complete the training, however there must be at least one staff member on site at all times that have completed the necessary training to administer this medication.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:10am and 3:00pm. At the start of the inspection there were a total of 82 children with 15 staff; all ratios were incompliance. A total of 5 children?s files and 5 staff files were reviewed. The children were observed playing outside on the two playgrounds, coming inside and washing hands, being read to and eating lunch (ham and cheese sandwiches, cooked carrots, applesauce and milk). Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #4 file does not contain a complete street address for their second emergency contact.

Plan of Correction: We will have mom fill it in today.

Standard #: 22VAC40-185-240-C
Description: Based on record review, staff that work directly with children have not obtained 16 hours of annual training. Evidence: Staff #4 file does not contain any documentation of training since 10/5/2017.

Plan of Correction: This staff member is waiting for her certificates from the training agency.

Standard #: 22VAC40-185-240-D-1
Description: Based on observation and interview, the center has agreed to administer a prescribed medication to two children without a staff member on site that has satisfactorily completed a training program that qualifies them to administer the medication. Evidence: Child #2 and Child #3 have emergency medication on-site to be administered in case of an accidental ingestion of an allergen. There are no staff members on-site that have completed the separate online training for this particular medication. Center management stated that they didn't know they needed to take an online training and thought that it was covered in the MAT training.

Plan of Correction: We will have all MAT trained staff complete the training.

Standard #: 22VAC40-185-270-A
Description: Based on observation, ares of the center were not maintained in a safe condition. Evidence: On the circular bike ride on the big playground, the handle bars of all of the bikes are heavily rusted.

Plan of Correction: We will sand and repaint the handle bars.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning and sanitizing materials were stored above food. Evidence: In the cabinet of the Penguins classroom, various cleaning materials are stored in a bin above food products.

Plan of Correction: The food items are used for art and cooking projects. I will have her move the food.

Standard #: 22VAC40-185-330-B
Description: Based on observation, playground equipment that the children could fall off of does not have a minimum of 6 inches of resilient surfacing. Evidence: Underneath the swings of the small playground there is approximately 1 inch of mulch. Underneath the swings, monkey bars and fire pole on the large playground, there is approximately 1 inch of mulch.

Plan of Correction: We have new mulch scheduled to be replenished next month.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and interview, the findings from the most recent inspection were not posted. Evidence: At the entrance of the facility, the program has the summary of the most recent inspection posted. When asked about the violation notice from the last visit, center management retrieved the violation notice from being stored behind the desk and stated that they didn't know it needed to be posted with the summary.

Plan of Correction: This was corrected during today's inspection.
