An unannounced monitoring inspection was conducted from 12:30pm -2:30pm. Upon arrival there were 31 children in care with 12 staff present. A sample of 3 staff and 3 children's records were reviewed in addition to 4 medications. Children were observed transitioning to afternoon classes and in circle time. The center does not offer transportation.

Standard #: 22VAC40-185-160-C Description: Based on a record review the center did not ensure all staff had a repeat tuberculosis screening 2 years since the last one. Evidence: 1. Staff #1's last tuberculosis screening was dated 09/20/16. 2. Staff #2 and #3's last tuberculosis screening was dated 09/21/16. Plan of Correction: The following corrective plan was provided by the Assistant Director: Staff will be sent out right away to have an updated TB done.
