22VAC40-185 SPECIAL SERVICES.
An unannounced monitoring inspection was conducted from 11:00am-2:30pm. At the time of entrance, 66 children were in care of 11 staff members present. Children were observed while engaged in group activities, during handwashing, lunch and transitions to naptime. Chicken strips, broccoli, pita bread and fruit salad were served today. A selection of 6 staff records, 8 children's records and other documents were reviewed. No children's medications were on site today. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A Description: Based on a sample review of records, 1 out of 8 children's records did not include the required information. Evidence: The record for Child #6 was missing documentation of viewing proof of the child's identity and age. Plan of Correction: We will add this information to the record.
Standard #: 22VAC40-185-70-A Description: Based on a sample review of records, 2 out of 6 staff records did not include written information to demonstrate that the individuals possess the education, certification, and/or experience required by the job position. Evidence: 1. The records for Staff #1, identified as a program lead, did not include documentation of certification/training required by the position. 2. The record for Staff #3, identified as a program lead, did not include documentation of education required by the position. Plan of Correction: 1. We will obtain documentation of the staff's child care training certificate. 2. Documentation of transcripts have been requested and will be added to the record once received.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: Several cleansers were observed in an unlocked cabinet in the kitchen that was open and accessible to children in care. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-185-420-E-3 Description: Based on a sample review of records, 1 out of 8 children's records did not include parent confirmation that the required information in the child's record is up to date annually. Evidence: The most recently documented update to the record of Child #1 was obtained 2/28/2017. Plan of Correction: We will have the parent review and update the record as needed.
Standard #: 22VAC40-185-440-E Description: Based on observation, occupied cots were not adequately spaced apart. Evidence: 4 occupied cots in the older toddlers classroom were not spaced at least 12 inches apart. Plan of Correction: Corrected during inspection; Staff rearranged cots.
Standard #: 22VAC40-185-510-J Description: Based on observation, medications were not kept in a locked place to prevent access to children. Evidence: An over-the-counter medication was observed in an unlocked upper cabinet in an an open and unloced office adjacent to the preschool classroom. Plan of Correction: Corrected during inspection; A lock was installed.
Standard #: 22VAC40-185-540-C Description: Based on observation, the first aid kit did not include the required emergency supplies. Evidence: The first aid kit was missing 2 required triangular bandages and a pair of tweezers. Plan of Correction: The purchase order for the missing supplies was placed during inspection.
Standard #: 22VAC40-185-570-A Description: Based on observation, the protective belt was not fastened while two infants were sitting in an infant feeding table. Evidence: During the inspection, Child #4 and #5 were observed sitting at an infant feeding table while the buckles were not fastened. Plan of Correction: Corrected during inspection; The staff fastened the belts.
