Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-240-D-4 (Medication training); 22VAC40-185-(4)-270-A (Building maintenance);

Comments:
An unannounced monitoring inspection was conducted on 10/4/18 from 9:30am - 12:45pm. When the Licensing Inspector arrived there were 77 children ages seven weeks old to four years old in care with 16 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch and during rest period. Records were reviewed for eight children and eight staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-510-G
Description: Based on a review of the medication stored at the facility, it was determined that the licensee did not ensure that in order to administer prescription medication, written parent authorization is obtained. Evidence: 1. The medication authorization for child #3 was signed by the doctor, however the medication authorization was not signed by the parent. 2. The medication authorization for child #9 was signed by the doctor, however the medication authorization was not signed by the parent. 3. The medication authorization for child #10 was signed by the doctor, however the medication authorization was not signed by the parent. 4. Staff #9 (Program Director) reviewed the medication authorizations for child #3, child #9, and child #10, and confirmed that neither parent signed the medication authorization.

Plan of Correction: The facility responded: The parents of child #3, child #9, and child #10, will be asked to sign the medication authorizations today (10/4/18). All new medication will not be accepted unless there is written parent authorization for the medication.
