Comments:
An unannounced monitoring inspection was conducted from 4:30 - 5:45 pm. There were 37 children, ranging in age from six years to 11 years of age, present with three staff supervising. The children were observed playing a group game, eating snack, having quiet time for homework, and participating in structured center play. Snack served today was a fresh apple and milk. Four children's records and two staff records were reviewed and in compliance. The emergency escape drill log was reviewed and in compliance. Two emergency medications and authorization forms were reviewed and in compliance. The first aid kit was observed and in compliance. Description: Based on review of two staff records, the center did not have an Office of Background Investigations (OBI) fingerprinting-based criminal history record determination letter prior to a staff beginning employment. Evidence: Staff A's (hired 8/13/18) record did not have a determination letter.

Plan of Correction: Staff had fingerprinting done but does not have the documentation on site. Will obtain this documentation immediately.
