The inspector conducted an unannounced mandated inspection from 11:00 a.m. until 1:30 pm. There were 12 children in care with staff supervising at all times. Children were observed participating in a fire safety exercise and being read to, playing outside, coloring, washing hands and eating lunch. No medication was reported to being on sight. Five children's records and six staff records were reviewed. The violations from the last inspection had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-240-C Description: Based on record review and interview, the center failed to have all staff complete 16 hours of annual training. Evidence: 1. The inspector reviewed six staff files. Staff #3 had completed six hours of training. Plan of Correction: The director will have staff complete the required hours of training in the future.
Standard #: 22VAC40-185-330-B Description: Based on observation and measurement, the center failed to have the required depth of resilient surfacing underneath and surrounding play equipment. Evidence: 1. The inspector measured the mulch around the slide and it was 2-4 inches deep and there was grass and weeds showing in the fall zone. Plan of Correction: The director said that the slide would be moved so that it would have the required use zone with resilient surfacing and that more mulch would be spread.
Standard #: 63.2-1721.1-B-2 Description: Based on record review and interview, the center failed to have all the required background check information in the file before beginning work. Evidence: 1. The inspector reviewed six staff records. Three out of six records did not have fingerprints prior to beginning work. Staff #4, staff #5 (date of hire 09/18/18, staff #6 date of hire 09/14/2018). Plan of Correction: The director will have the staff complete the fingerprint check with ASAP.
