Comments:
An unannounced inspection was conducted on 6/25/2018, a follow up visit with another local agency was conducted on 7/10/2018 and on 8/23/2018 in response to a self-reported incident that occurred on 6/19/2018 regarding forbidden actions. Based on interviews conducted, observation and documentation reviewed, violations were cited and are identified on the Violation Notice. Violations:
Standard #: 22VAC40-185-410-1
Description: Based on observation and interviews, a staff member did not use behavioral guidance that was constructive in nature, age and stage appropriate, when trying to redirect a child to appropriate behavior. Evidence: In the school age classroom, staff#1 stated that they grabbed/held child#1(9 years old) by the shirt on the right shoulder area, and walked the child over to a table approximately 4 feet away from where the child was standing, once they were both close to a chair, staff#1 used both arms to forcibly sit child#1 down. Staff#1 stated that child#1 was throwing toys, kicking chairs and hitting children, and they felt this was the only way they could protect the other children in the classroom.

Plan of Correction: Teacher will only be placed in a classroom that matches her interests and abilities, age up to 2 years only. Retraining was completed on 8/22/2018. Positive Behavior Support, Quality Foundations, and NAEYC's Educational Excellence guide to Accreditation. Continue to observe and demonstrate accepted approach to child guidance. Review behavior guidance in all day training to reinforce.
