Comments:
An unannounced subsidy health and safety inspection was conducted in conjunction with a Licensing Monitoring Inspection. There were 45 children with 4 staff. Children were observed upon arrival, going outside, having snack and homework time. Reviewed staff files - see violation notice Reviewed 10 children's files - complete Reviewed medications for 8 children and allergy action plans for 6 - complete Emdergency drills including Lockdown were complete If there are any questions please contact Linda Reynolds at 540-272-2663 or Linda.reynolds@dss.virginia.gov

Standard #: VENDSUB-000-012-B
Description: Based on a review of 6 staff records, it was determined that documentation of Virginia Pre Service Training wasd not in two files. Evidence: The Virginia PreService Certificate was no in the files for Staff 6 and staff 5, both of whom were employed last year and had files checked in the Spring.

Plan of Correction: The Director will get copies of the training certificate from the office.
