63.2 Licensure and Registration Procedures

An unannounced monitoring inspection was conducted on October 15, 2018 from approximately 2:10 pm until 4:15 pm. At the time of entrance there were 13 children and 4 staff members were present with children. There were additional staff on site for administrative purposes. The sample size consisted of 5 children's records and 3 staff records. Three medications were reviewed. Children were observed eating snack and during story time. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the site manager. Standard #: 22VAC40-185-240-C Description: Based on record review and staff interviews, the licensee did not ensure program directors and staff who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: On October 15, 2018, the following was found during record review: 1. The record for staff #2 only included documents for review of 4 hours of annual training. 2. The record for staff #4 only included documents for review of 3 hours of annual training. Plan of Correction: The center coordinator will track training moving forward.
Standard #: 22VAC40-185-380-A-2 Description: Based on observation and staff interviews, the licensee did not ensure centers operating five or more hours per day shall have a designated rest period for at least one hour but no more than two hours. Evidence: On October 15, 2018, in room #12 two children, child #2 and #3 were observed sleeping at 2:45 pm. The center staff said the children, including child #4 and #5 laid down at 11:30 am. The classroom schedule states rest period is from 12 until 2. Center staff said it was their procedure not to wake children when they have fallen asleep during their rest period. Plan of Correction: The center coordinator will retrain staff.
Standard #: 22VAC40-185-500-B Description: Based on observation and staff interviews, the licensee did not ensure a nonabsorbent surface for diapering or changing shall be used. Evidence: On October 15, 2018, in room #13 the blue diaper had a rip approximately 1 inch long and the foam was exposed. The diaper pad was not longer nonabsorbent. Plan of Correction: The center coordinator has ordered a new pad and waiting for it to come in and will use another pad in the meantime.
Standard #: 63.2-1720.1-B-2 Description: Based on record review and staff interviews, the licensee did not ensure 1 of 3 staff records has a Office of Background Investigations (OBI) letter stating eligibility for hire in response to the national fingerprint check prior to beginning employment. Evidence: On October 15, 2018, the record for staff #1 (start date 9-17-18) has a OBI letter of eligibility for hire dated 9-21-18. Plan of Correction: The center coordinator is working with HR.
