Comments:
An unannounced monitoring inspection was conducted today from 10:36am-2:00pm. There were 59 children (3-5yrs.) supervised by 8 direct care staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating snack brought from home, completing worksheets, playing board games, in home-living, listening to audio books, exploring in the discovery table, and creating pictures with shapes. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, 1 of 5 staff records did not contain documentation of a current TB test or screening, conducted every two years from the date of the first initial screening or test. Evidence: Staff A's (date of hire: 8/31/2015) most recent documented TB test was dated 9/27/2016 (expired: 9/27/2018).

Plan of Correction: We will have the employee obtain a new TB screening.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: In the Fuzzy Bears classroom (children 3 years old), an unlocked cabinet above the sink contained a container of clorox wipes and lysol aresol spray.

Plan of Correction: We will place the cleaning supplies in a locked cabinet.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, 1 of 5 medications did not comply with methods to prevent use of outdated medication. Evidence: Child #1's (start date: 9/5/2017) Children's Benadryl expired 3/2018.

Plan of Correction: We will return the expired medication to the family and request a new medication.

Standard #: 22VAC40-185-560-G
Description: Based on a sample review of lunch boxes, all food containers were not clearly dated and labeled in a way that identifies the owner. Evidence: 1. In the Shooting Stars classroom (children 4 years old), approximately 3 of 7 lunch boxes were not labeled with the current date. 2. In the Rainbows classroom (children 4-5 years old), approximately 5 of 10 lunch boxes were not labeled with the children's names.

Plan of Correction: We will remind the parents to label all lunch boxes with first and last names, and dates.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 5 staff records did not contain documentation of central registry findings within 30 days of employment. Evidence: Staff D's (date of hire: 8/3/2018) record did not contain documentation of of central registry results.

Plan of Correction: We will reach out to the diocese office to obtain a copy of the CPS check.
