An unannounced monitoring inspection was conducted from 2:00pm-3:30pm. At the time of entrance, 50 children were in care with 3 staff members present. Children were observed during check in and free play. A selection of staff and children records, medications, evacuation drill log and other documents were reviewed. All three Staff on site are certified in CPR and first aid training. Staff files were reviewed on an earlier date. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A Description: Based on record review, one of five children's records did not have complete information. Evidence: 1. Child A's file was missing one parent's work phone number and previous child day care and schools attended by the child. Plan of Correction: I will ask the parent for the information.
