Comments:
An unannounced monitoring inspection was conducted on October 9, 2018 from 11:20 am until 1:15pm. Upon arrival to the facility, there were 74 children present with 15 staff members. The children were observed participating in age appropriate activities, hand washing, diapering and napping. A sample of 7 children's records and 7 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There was a violation cited in the area of physical plant. An exit meeting was conducted with the facility director prior to the end of the inspection.

Description: Based upon observation and interview the provider did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: 1. During the inspectors tour of the facility it was found that the outdoor school age playground structures protective coating was peeling, exposing rusting metal and sharp metal edges on the bridge and stairs of the playground structure. a. The exposing rusting metal and sharp metal edges on the bridge and stairs of the playground structure could be a potential injury hazard to children. b. Staff #3 toured the school age playground with the inspector and also identified and confirmed the violation.
