Comments:
An unannounced monitoring inspection was made on 10/17/18. There were 49 children present with six staff supervising. Five children's records were reviewed. There were five medications observed. Six staff records were reviewed on 10/19/18 and the inspection was completed on that date. The inspector arrived at the school at 2:45 pm and departed at 5:30 pm. The children were observed at arrival, during outside time, during snack, during homework time, and during departures. There was discussion with the site coordinator about the following: supervision of children by staff in several scenarios observed during the inspection, children's arrival from another school (getting off the bus coming into the program).

Violations:
Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that the children washed their hands as required. Evidence: 1. The children were observed during snack. The children did not wash their hands after eating snack and did not use disposable wipes after eating.

Plan of Correction: The site director will remind all staff of the requirement to use wipes after eating snack.

Standard #: 22VAC40-185-510-J
Description: Based on observation and interview, the center failed to ensure that medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A bag was observed on a cafeteria table which was labeled as having an Epipen and Benadryl. The bag was not locked using a safe locking mechanism to prevent access by children. The bag was a cloth bag with a clasp on it that can be opened by pinching the sides to relase the clasp. The bag was accessible to all of the children. 2. The bag was observed to have an Epipen, a bottle of liquid Benadryl, and an inhaler with albuterol inhalant solution in it. 3. The site director was asked about the contents and stated that the medications are in the bag for the child to carry. The child for which the medications were prescribed is six years old.

Plan of Correction: The site director will make sure that the medications are locked at all times unless otherwise authorized by a written physician's order to have them unlocked.

Description: Based on observation, the center failed to ensure that the procedures for emergency evacuation and shelter-in-place were posted. Evidence: 1. The evacuation and shelter-in-place map was observed to be posted. The procedures were not observed for the two types of emergency practice drills.

Plan of Correction: The site director will get a copy from the child care director and post them.

Description: Based on observation and interview, the center failed to ensure that the menu for the center provided food was posted. Evidence: 1. The center provides an afternoon snack. The menu for September was observed to be posted. 2. The site director stated that the October menu had fallen off the board it was posted and could not be found.

Plan of Correction: A menu for the rest of the month will be posted by next week.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the center failed to ensure that the findings from the most recent inspection of the facility were posted. Evidence: 1. The "Violation Notice" was not observed to be posted.

Plan of Correction: The site director will post the results from this inspection today.
