A Subsidy Health and Safety inspection was conducted on 10/18/2018 to review supplemental health and safety requirements. There were 41 children, ages 7 weeks-4 years, and 8 staff providing direct care and supervision present during today's inspection. The inspector reviewed 5 children's, 5 staff records and 3 medications during today's inspection The children were observed in the following activities: finishing up lunch, during rest time, having an afternoon snack and in free choice play. Proper hand washing, table sanitization and diapering were also observed during the inspection. Infants were observed being fed and allowed to sleep on demand. The inspector reviewed the emergency procedures and emergency supplies during the inspection today. The inspector discussed the following with the site staff: attendance requirements, children's allergies, background check requirements, model forms for subsidy providers, and caring for infants. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety inspection were found and are noted on the Violation Notice. Standard #: VENDSUB-000-006-B-11 Description: Based on record review and discussion with the staff, the facility failed to obtain a written allergy care plan for each chid with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child #2 had documentation of a diagnosed food allergy. There was not a written allergy care plan for Child #2 on record. Plan of Correction: Allergy Care Plan form will be given to parents today.
