Comments:
The inspector conducted an unannounced renewal inspection from 2:30 p.m. until 4:30 p.m. There were 11 children present upon arrival and nine more children arrived after school. Three staff were supervising at all times. Children were observed during a diaper change, waking up from a nap, having a snack, being read to, playing, and watching a video. Parents were observed picking up their children. Staff were observed getting children off of the head start bus and the public school bus. No medications were reported to be on site and/or being administered. Five children's records were reviewed and three staff records were reviewed. The violations from the last inspection had been corrected. Violations are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Description: Based on observation, the center did not maintain the inside of the center in a safe manner. Evidence: 1. The inspector observed a plugged in candle with hot wax on a counter accessible to children.

Plan of Correction: Corrected during the inspection.
