Comments:
An unannounced inspection was conducted today from 10:20am to 11:40am in response to a self-reported incident which occurred on October 10, 2018. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interviews and review of documentation, the center failed to follow its own policies and procedures for supervision. Evidence: The Child Supervision Record (Name-to-face) documentation was not used correctly by Staff #1 when transitioning from the playground back into the classroom.

Plan of Correction: On the day of the incident, Staff #1 scheduled a one on one review of the policies with the director. The meeting was conducted the following day. The entire staff was also given a review of the attendance/transition policies.

Standard #: 22VAC40-185-340-A
Description: Based on interviews and documentation review, staff failed to ensure the care and protection of a two year old child. Evidence: Child #1 was left alone on the playground as the class and Staff #1 entered the building through the outside classroom door. The incident happened when transitioning from morning outside play. Staff #2 entered the classroom and immediately observed the child was missing and noticed the child peering through the window. The child was immediately retrieved from the playground, unharmed.

Plan of Correction: The incident was immediately reported to management. Upon review, the estimated time the child was alone was under one minute. Appropriate actions were taken and the following were contacted: parents, next level supervisors, the corporate office and the Virginia Department of Licensing. The child was unharmed and joined the class.

Description: Based on staff interviews and documentation review, a two year old child was left alone on the playground and not always within sight and sound supervision of staff. Evidence: Child #1 was left alone on the playground as the class and Staff #1 entered the building through the outside classroom door. The outside door closed, leaving the child on the playground. The incident happened when transitioning from morning outside play. Staff #2 entered the classroom and immediately observed the child was missing and noticed the child peering through the window.

Plan of Correction: The child had been out of site and sound for less than one minute. The child was immediately retrieved from the playground, unharmed. Transition and supervision training, as well as attendance reviews, were completed by Staff #1 one-on-one with the director. The entire staff also completed a review of the training by the end of the week.
