Technical Assistance:
Discussed ratios of children based on ages, during naptime and while outside on the playground.

Comments:
An unannounced renewal inspection was completed today. The physical plant, 5 staff files, 5 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 12:30-2:45. There were a total of 45 children with 9 staff; all ratios were in compliance. For lunch, the children were served whole grain pasta with marinara sauce, cheese, peas, banana and mlk. The children were observed resting during naptime, listening to music and playing with the caregivers. Standard #: 22VAC40-185-140-A
Description: Based on file review, 2 out of 5 children's records do not have physical examinations. Evidence 1. Children #3 and #4 do not have physicals on file.

Plan of Correction: We will ask the parents for these.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain subsequent tuberculosis (TB) test results every two years. Evidence 1. Staff #1's most recent TB test results are dated 8/17/11. Staff #3's most recent TB test results are dated 3/25/14.

Plan of Correction: Staff #1 has completed this, but does not have the documentation. She will obtain this. Staff #3 will complete this.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 2 out of 5 staff records do not have all required information on file. Evidence 1. Staff #2 and #3 do not have written information to demonstrate that the individual possesses the education required by the job position. 2. Staff #3 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment.

Description: Based on observation, infant bottles were not labeled with today's date and/or a child's name. Evidence: 1. The refrigerator in the Older infant classroom contained 1 bottle that did not contain a name or date

Description: Based on file review, 1 out of 5 staff records reviewed does not have a search of the central registry (SCR) completed within 5 years from the date of the last check. Evidence 1. Staff #1's most recent SCR was completed on 12/7/12.
