Comments:
Today an unannounced monitoring inspection was conducted between the hours of 3:05pm and 5:30pm. I observed 33 children with 2 staff. Ratios were in compliance. Children were observed playing table games, doing crafts, doing homework, and eating snack. Today's snack consisted of Cinnamon Roll and CapriSun 100% apple juice. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. There were no medications during today's inspection. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation of immunizations for each child in care before the first day of attendance. Evidence: Child#3's record did not contain documentation of the child's physical examination.

Plan of Correction: We will follow up with the parents.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the program did not have a physical examination by or under the direction of a physician for each child in care within 30 days after the child's first day of attendance. Evidence: The records of Child#2 with an enrollment date of 4/19/2018, and Child#3 with an enrollment date of 8/23/2018, did not contain documentation of a physical examination, the record of Child#4 with an enrollment date of 8/15/2018 is missing the first two pages of the physical examination.

Plan of Correction: We will follow up with the parents.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: The records of Child#1, #2, #3, and #4 are missing the parents' place of employment. Child#5's record is missing the parents' place of employment, the address of one of two required emergency contacts and the street number of the second emergency contact.

Plan of Correction: We will follow up with the parents.

Standard #: 22VAC40-185-530-A
Description: Based on record review and staff interviews, the program did not ensure there was at least one staff member trained in first aid, cardiopulmonary resuscitation and rescue breathing (CPR) as appropriate to the age of the children in care during the program's hours of operation. Evidence: There were two staff members present during the inspection, neither one of the staff members had a current first aid/CPR certification during the inspection. Staff#1 has a CPR/First Aid certification that expired on 5/25/2018.

Plan of Correction: We are waiting to be scheduled on the next class.

Standard #: 22VAC40-185-540-E
Description: Based on review of emergency supplies, not all emergency supplies were in operable condition. Evidence: The program did not have one working, battery-operated flashlight during the inspection.

Plan of Correction: We will get a new flashlight.

Standard #: 22VAC40-185-550-B
Description: Based on review of procedures for emergencies, the emergency preparedness plan did not contain required procedural components. Evidence: The program's emergency evacuation plan does not include assembly points. The program's shelter-in-place plan does not include primary and secondary means of access and egress.

Plan of Correction: We will review and update emergency plan.
