22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 8:15am - 11:15am. Upon arrival there were were 56 children in care with 13 staff present. A sample of 5 children and 4 staff records in addition to 2 medications. Children were observed playing in centers and being read a story. Classes were beginning to get ready for lunch. Children were also observed entering the building.
Standard #: 22VAC40-185-60-A Description: Based on observation the center did not maintain the required information for each child enrolled. Evidence: Child #1 and #4 did not have a complete address for the 2 required emergency contacts. Plan of Correction: This was corrected on 10/15. All children must have a complete file before they are allowed to start. Management will do periodic checks to ensure compliance.
Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: There was peeling paint on the gym wall. Plan of Correction: This will be completed by 11/14/2018. The safety captain will be doing daily walk through to ensure compliance and will report any deficiencies within 24 hours for management to handle.
Standard #: 63.2-1720.1-B-3 Description: Based on a record review the center did not ensure all required out-of-state central registry checks were complete. Evidence: Staff #3 listed living out of state within the last 5 years on the most recent sworn statement of affirmation. The center did not send for the central registry check in the state staff #3 lived in. Plan of Correction: This was corrected on 10/15. All out of state employee's CAN's will be submitted day one of hire to ensure compliance and all correspondence will be documented for all follow-up within 30 days. No new employee will start without a complete file on day one. There will be periodic staff file checks by management team. There will be a DM inspection periodically to ensure compliance.
