Comments:
An unannounced renewal inspection was conducted from 9:30 am-12:00pm. At the time of entrance, 59 children were in care with 12 staff members present. Children were observed during snack time, singing at circle time, playing in centers, dancing with musical instruments, listening to the teacher read a book and engaged in group activities. Positive interaction was observed between the children and teachers. A selection of staff and children records, medications, first aid kit, emergency drills and other documents were reviewed. Standard #: 22VAC40-185-50-A
Description: Based on observation, the center did not treat children's records confidentially. Evidence: 1. The supplemental page dated 3/20/2018 and 9/15/2017 were attached to the bulletin board in the Director's office and should be kept confidential and not posted.

Plan of Correction: The supplemental pages were removed immediately.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. In the PreK-2 classroom, a brown chair pad had a large rip approximately 2 inches long in the material.

Plan of Correction: I removed the pad immediately and will replace it.
