Standard #: 22VAC40-185-130-B
Description: Based on review of children's records, the center did obtain documentation of additional immunization once every six months for children under the age of two. Evidence: Children # 1 and 2 do not have documentation of additional immunizations, last update was 1/4/18 and 4/9/18 respectively.

Plan of Correction: I will get an update record from the parents.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the center does not maintain complete records. Evidence: Staff #1 and 3 do not have documentation of their education or qualifications for the position. Staff #2 does not have documentation of references being checked prior to employment.

Plan of Correction: I will make sure the records are complete.

Standard #: 22VAC40-185-500-A
Description: Based on observation, hands are not being washed with soap and water. Evidence: Infant's hands were not washed after diaper changes. Children's hands were not washed before eating snack.

Plan of Correction: I will review our hand washing policy with all staff.

Standard #: 22VAC40-185-500-B
Description: Based on observation, diapering surface is not being cleaned and sanitized after each use. Evidence: The diaper surface is being sprayed with soap and water and wiped dry, but not sanitized with bleach and water and left to air dry.

Plan of Correction: I will review the cleaning and sanitizing procedure with all staff.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications and authorizations, medication was not returned to parent or disposed of within 14 days of medication authorization. Evidence: Child #1 has documentation of medication authorization that expired 9/12/18, whereas the medication has not been returned to parent or disposed of.

Plan of Correction: I will contact the parent to get an updated form.

Standard #: 22VAC40-185-560-K
Description: Based on observation, tables were not sanitized before each food was served. Evidence: Tables the children were using for activities, were not sanitized before snack was served.

Plan of Correction: I will make sure they spray the table before any food is served.
