22VAC40-191 Background Checks (22VAC40-191)

An unannounced renewal inspection was conducted on October 16, 2018, between the hours of approximately 10:25 a.m. and 2:25 p.m. Fifty-one (51) children were enrolled and thirty-three (33) children were in the care of seven staff members, in four classrooms, during the on site inspection. Two additional staff members were present and assisted, as needed. Five children's records were reviewed and five staff records were reviewed and/or updated. Three children's medications, two children's topical ointments and accompanying authorization forms were reviewed. Safety equipment, posted information (license, inspection documentation, weekly menu, emergency evacuation diagram/plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drill/shelter-in-place drill records, local agency reports, insurance verification, injury reports, infants' daily sheets and attendance records) were observed/reviewed. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities provided on this date included: indoor play - toys, stories, concept development (colors); restroom, diapering and handwashing procedures; lunch (peanut butter and jelly sandwiches, carrots, applesauce and milk); return of children from a fieldtrip; naptime; infant care; afternoon snack (cheese, crackers and water) and arrival/departure of children. Previously cited violations were reviewed for compliance; no repeat violations were cited. Exceptions to compliance were noted on the accompanying Violation Notice. Standard #: 22VAC40-185-280-B
Description: Based on observation and discussion with staff on October 16, 2018, the program failed to ensure cleaning supplies were kept in a locked place using a safe locking method that prevented access by children. Evidence: 1. On this date, the licensing inspector (L.I.) observed hazardous substances/cleaning supplies (Windex, Lysol Toilet Bowl Cleaner and Clorox Disinfecting Wipes) stored in a plastic container, on the floor, in an unlocked supply closet. A container of Air Freshener was stored on a shelf in the closet. The labels on the hazardous substances/cleaning supplies contained the terms "Caution," "Danger," and "Hazard." 2. Staff #1 confirmed the supply closet, containing hazardous substances/cleaning supplies, was unlocked.

Plan of Correction: The supply closet was immediately locked. Staff will be reminded to lock the supply closet.

Standard #: 22VAC40-185-510-G
Description: Based on observation/review of three children's medications and discussion with staff on October 16, 2018, the program failed to obtain prescription labels for two children's medications and failed to ensure a MAT (Medication Administration Training) trained staff member was on site/available to administer emergency medication, as needed. Evidence: 1. Observation of medications for child A and child F, on this date, determined that accompanying prescription labels were not on site/available. 2. As observed, on this date, a MAT trained staff member was not on site/available for approximately one hour, to administer emergency medication, if needed. One child, requiring emergency medication, was present on this date. 2. Staff #1 confirmed the lack of prescription labels for the children's medications and the lack of a MAT trained staff member on site/available to administer medication, if necessary.

Plan of Correction: The children's parents will be contacted and prescription labels will be obtained for the medications. An additional staff member is scheduled for MAT training. A MAT trained staff member will be on site, as needed, to administer medication(s).
