Licensing inspector discussed developing a plan for non-communicable diseases with director.

An unannounced investigation was conducted on 08/08/2018 in response to a complaint regarding special care provisions/ emergencies and physical plant. During the investigation, 8 staff members and 3 parents were interviewed and documentation was reviewed. The preponderance of evidence does not support the allegation; therefore, the complaint is deemed not valid. The exit interview was conducted with the director. Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on record review, children's records were incomplete. Evidence: 4 of 16 records reviewed were missing required information. 1) Child A was missing 1 of 2 required emergency contacts. 2) Children B, C and D were missing 2 of 2 required emergency contact addresses. Plan of Correction: Files are in the process of being updated to reflect the required information.
