The Licensing Inspector conducted an unannounced inspection on 10/15/18 in response to a complaint that was received in the Licensing office on 10/10/18 concerning infant feeding. Observations were conducted, staff were interviewed, and documentation was reviewed during the investigation. Based on the information gathered during the investigation, the evidence supports the complaint and is deemed to be valid. The area of non-compliance associated with the complaint is on the attached violation notice.

Standard #: 22VAC40-185-340-A Complaint related: No Description: Based on interview, it was determined that the licensee did not to ensure that when staff are supervising children they shall always ensure their care and protection. Evidence: 1. On October 9, 2018, child #1 was fed three ounces of breast milk that belonged to child#2. 2. Staff #2 (Assistant Director) acknowledged that child #1 was fed breast milk from child #2 instead of formula. Plan of Correction: The facility responded: Management immediately separated from employment the staff member who administered the incorrect bottle, and retrained all staff on company policy concerning the proper administration of bottles.
Standard #: 22VAC40-185-560-G Complaint related: No Description: Based on interviews, it was determined that the licensee did not ensure that when food is brought from home it is labeled with the child's name and date. Evidence: 1. During interviews, staff #3 stated that on October 9, 2018 child #1 and child #2 both had bottles were not labeled with their name or the date. 2. Staff #2 (Assistant Director) acknowledged that based on their investigation that on October 9, 2018 the bottles for child #1 and child #3 were not labeled with their name or the date. Plan of Correction: The facility responded: Brilliant Beginnings Staff will continue to implement their policy requiring parents supplying bottles to label them with their child?s name, date and contents. Further, bottles containing breastmilk must be contained in a closed plastic bag. Staff have been retrained on this policy and will not accept any bottles that have not been pre-labeled (as staff in the past have labeled bottles for parents who?ve forgotten). Bottles must meet the aforementioned criteria. Management is purchasing color coded labels to better identify bottles. The bottle refrigerator is organized in a manner so that formula bottles are stored in one area, and breast milk bottles are kept in another area. Additionally, detailed information and pictures are now posted on the bottle refrigerator delineating each child?s bottle for caregivers to reference if there ever is a question. Finally, the refrigerator shelving is now labelled with each child's name to further ensure staff knowledge of which bottle belongs to who.
