Reviewed the Child Care Program Regulation (22VAC40-665) - Subsidy inspection requirements memo and Group Size Requirements Information Sheet that was distributed to subsidy providers on 10/16/2018. Left a copy of memo and information sheet with the provider.

The licensing inspector conducted a Subsidy Health and Safety inspection Wenesday, October 24, 2018 from 09:30 am to 2:00 pm to review supplemental health and safety requirements. The children were observed engaged in a variety of activities ranging from playing at the on-site gym, listening to stories during large group time, and playing in the learning centers. The infants were observed sleeping in cribs and playing with toys on the floor while their teacher read a story. Nine children?s records and nine employee records were reviewed during this inspection. Medication is being administered, however none was present The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/03/2018 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 05/30/2017 Total enrollment: 97 Today, the following child to staff ratios were observed: Infants I ? 12:3 Infants II-11:3 Toddlers ?10:2 Early Preschool (Twos) ? 19:3 Threes ?25:3 Pre-K ?18:2 School Age ?not present A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements were found and are identified on the violation notice. New subsidy requirements became effective on 10/17/2018. The facility has not yet complied with the following requirement: 22VAC40-665-770-Procedures for emergencies (Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must be included in the emergency response plan.) was provided. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on a review of staff records and staff interview on 10/24/2018, the center did not ensure that all staff providing care to children shall have current certification in first aid and cardiopulmonary resuscitation (CPR) by the required time frames. EVIDENCE: 1. The record of Staff #4, whose date of hire is 08/16/2018, did not contain documentation of current certification in first aid and cpr. 2. The record of Staff #6, whose date of hire is 04/02/2018, did not contain documentation of current certification in first aid and cpr. 3. The record of Staff #7, whose date of hire is 08/15/2018, did not contain documentation of current certification in first aid and cpr. 4. The record of Staff #9, whose date of hire is 08/23/2018, did not contain documentation of current certification in first aid and cpr. 5. Staff #1 stated that these staff did not have first aid or cpr.
