We discussed the requirements for medication labeling and updated TB statements. The risk assessments for violations were provided. We talked about the posting of menu substitutions.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 1:25 PM to 7:25 PM. Today, there were one hundred and eighteen children, ages infancy through four years, in after care with thirty-two staff. Ten school age children and seven preschool age children received evening care with two to four staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, equipment, agency inspections, posted information, emergency planning, injury records, emergency supplies and drill records, nutrition, snacks/dinner, medication, diapering procedures, ten records for children, ten staff records, staff qualifications, staff training, indoor and outdoor areas. Today, I viewed changes and additions on two playgrounds. The new playground equipment, shade area, surfacing, basketball area and a tricycle path were determined to meet safety requirements. Some children took a walk during the afternoon. Inside, there was a large selection of age appropriate toys, equipment and materials. Staff were observed providing appropriate supervision and positive interactions with children. The staff have participated in training opportunities related to the care of children. Standard #: 22VAC40-185-160-C Description: Based on a review of ten records for staff, and interviews with administrative staff, the updated tuberculosis screening and statement was not obtained by two years from the date of the previous statement for one staff. Evidence: The updated tuberculosis screening and statement completed on July 25, 2016 for staff member 9 was not updated until August 16, 2018. Administrative staff indicated that arrangements for TB screenings by a health care provider were made for staff needing TB statements at the center for August. Plan of Correction: In the future, administrative staff will inform staff in advance about due dates for updated TB statements. All staff will be expected to arrange for TB screenings so that they submit statements by two years from the date of the last statement.
Standard #: 22VAC40-185-510-G Description: Based on a review of four prescription medications kept at the center, and interviews with staff, there was an emergency medication that was not labeled as required. Evidence: The emergency medication for child 2 did not have a prescription label with the child's name and the prescriber's instructions. Plan of Correction: The administrative staff will make sure that the parent is contacted to arrange for the parent to bring a box with the prescription label attached for storage of the emergency medication for child 2. The M.A.T. certified staff who accepts medication for children will always match the name of the child and a prescription label with the authorization form and always require the original labeled container.
