Comments:

An Unannounced Renewal Inspection was conducted on October 30, 2018. Upon entrance there were 13 children in care and 4 staff present with children. The inspection began at approximately 8:20 am and ended at 11:10 am. The record review sample consisted of 4 staff records and the background checks for the owner. There are no medications being administered at the center. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Standard #:	22VAC40-185-60-A
Description:	Based on record review of 5 children's records and staff interviews, licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which shall contains all information. Evidence: On October 30, 2018, the following records were reviewed and determined incomplete; 1) The record for child #1did not include the house number for the emergency contact. 2) The record for child #3 did not include the employment location or phone number for one parent listed. 3) The record for child #4 did not include the employment phone number for one parent listed or a second emergency contact name, home address or phone number.
Plan of Correction:	The center director will have parents update the records.
Standard #:	22VAC40-185-280-B
Description:	Based on observation, licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: On October 30, 2018, in the following areas chemicals were observed in unlocked cabinets. 1) In the fours room there was a cabinet with 1 container of sanitizer (bleach/water), disinfectant cleaner and glass cleaner. 2) In the toddler room there was a cabinet with 1 container of sanitizer (bleach/water), disinfectant cleaner and room deodorizer. The glass cleaner and room deodorizer were labeled "keep out of reach of children", and at least one of the other statements 'flammable", "danger" and "hazardous".
Plan of Correction:	The center director will lock the chemicals.
Standard #:	22VAC40-185-570-A
Description:	Based on observation and staff interviews, licensee did not ensure when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: On October 30, 2018, child #1 was observed sitting in a highchair with a blue cover without a safety strap.
Plan of Correction:	The center director removed the chair during the inspection.
