The new subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement(s) 22VAC40-665-770 Procedures for emergencies: Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must be included in the emergency response plan. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.
An Unannounced Subsidy Health and Safety Inspection was conducted on October 30, 2018. Upon entrance there were 13 children in care and 4 staff present with children. The inspection began at approximately 8:20 am and ended at 11:10 am. The record review sample consisted of 4 staff records and the background checks for the owner. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Standard #: VENDSUB-000-005-B Description: Based on observation and staff interviews, the vendor did not ensure for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: During the inspection on October 30, 2018, there was not documentation of daily attendance in the two's and three's room. Plan of Correction: The center director will remind teachers to documentation and add only necessary information to the clipboard.
Standard #: VENDSUB-000-012-D Description: Based on record review and staff interviews, the vendor did not ensure orientation training for staff shall be completed on the following facility specific topics (shaken baby syndrome, safe sleep, food allergies and emergency preparedness plan) prior to the staff member working alone with children and within seven days of the date of employment. Evidence: On October 30, 2018, the center director confirmed staff #2 (date of hire 8-22-18) and staff #3 (date of hire 9-27-18) did not have facility orientation on shaken baby syndrome and safe sleep. Plan of Correction: The center director will train the staff and add it to the orientation.
