Comments:
An unannounced monitoring inspection was conducted on October 31, 2018 from 9:30pm until 11:15am. Upon arrival to the facility, there were 60 children present with 13 staff members. The children were observed participating in age appropriate activities, outdoor play, hand washing and diapering. A sample of 6 children's records and 6 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There was a violation cited in the area of special feeding needs. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-570-E
Description: Based upon observation and interview the provider did not ensure that the prepared infant formula shall be refrigerated, dated and labeled with the child's name. Evidence: 1. During the inspectors tour of the facility it was found that the infant formula prepared for Child #5 and Child #6 located in the infant room refrigerator was not labeled with the current date. a. Child #5 and Child #6 were in attendance in the infant room during the time the violation was observed. b. The staff #6 toured the infant room with the inspector and also identified and confirmed the violation.

Plan of Correction: Staff will verify that infant formula bottles are date and labeled.
