22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced renewal inspection on 10/30/2018 from 10:05am to 4:45pm. The children were observed playing on the playground, participating in story time and singing. A formal observation was completed with the infant and three year old classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with the children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. Twelve injury reports were reviewed as well as the center?s injury prevention plan which was updated 09/4/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/25/2018 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 02/1/2018 Last health inspection: 8/24/2018 Today, the following child to staff ratios were observed: Infants - 3:1 Toddlers ? 14:4 Twos - 14:2 Twos - 5:1 Threes - 9:1 Threes and Fours - 8:1 Fours ? 10:2 Fours ? 10:2 Fours ? 9:2 Fours ? 9:2 School Age - 10:1 School Age - 13:1 School Age - 10:1 School Age - 14:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The inspector observed exposed protruding nails and sharp splintered wood on the fence surrounding the playground. Plan of Correction: Splintered wood is going to be replaced and the nails will be removed this Saturday and Sunday.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The inspector observed an inoperable step trash can used for soiled diapers in the rest rooms connected to room #5 and room #9. Plan of Correction: The step cans will be replaced tonight. Teachers will be informed of their responsibility to report broken equipment in their rooms.
Standard #: 63.2-1721.1-B-2 Description: Based on record review, the center did not ensure that results of a fingerprint check were obtained by 09/30/2018, for one applicant/licensee, hired prior to 01/22/2018. Evidence: The record of applicant #1 contained, fingerprint check results dated 10/15/2018 and documentation that the applicant was hired prior to 01/22/2018. Plan of Correction: Fingerprints will be renewed every 5 years promptly.
