The licensing inspector completed a scheduled modification study on Friday, November 2, 2018 from approximately 9:30 AM until 11:05 AM. The Department received a request to modify the facility?s license on October 4, 2018. The licensee has requested to relocate to a building located at 52 Walnut Street, Warsaw. The licensing inspector completed an inspection of the building located at this address and was assisted by members of management. One classroom has been designated as the area to be used by the children. A fenced area outside adjacent to the building will be used for an outdoor play space. All areas of the facility were inspected. The classroom has the required furniture and age appropriate materials for the anticipated number of children in care. All required information was posted. The inspector reviewed changes to the policies, measured the classroom, inspected the bathrooms and outdoor play areas. No hazards were identified, and no violations were cited during the inspection. At this time, the facility?s license will be modified to allow 17 children to be cared for at this facility. The children will be 3 to 5 years of age. The effective dates of the license will not be affected. If you have any questions, please contact the inspector at (804) 662-9790.
