An unannoucned renewal inspeciton was completed on 11/7/18. There were 156 children present . Ten children's records, 13 staff records, and five Board officer/agent records were reviewed. Six medications were also reviewed. The inspector arrived at the center at 8:00 am and departed at 4:00 pm. The children were observed during arrivals, during breakfast, during outside times, during free time/free choice activities, during lunch, and during nap. Infant activities were observed. Four classroom observations and two outside observations were made. There was discussion with the director about the following: the renewal application has not been received (discussion on the new application form), having the Board officers/staff complete the new Sworn Statement forms, practicing lock down drills in addition to the two shelter-in-place drills.

Description: Based on review of inspection reports, the center failed to ensure that there was annual fire inspection report from the fire official having jurisdiction. Evidence: 1. The last fire inspection report was completed 4/19/17.

Plan of Correction: This was corrected during the inspection. The director called the fire official during the inspection and the fire inspector arrived during the inspection to complete it.

Description: Based on review of inspection reports, the center failed to ensure that there was annual approval from the health department for water supply and sewage disposal system. Evidence: 1. The last "Report of Environmental Sanitation Inspection" form was completed 10/2/17. This report is required to be completed by the health department annually. The last food inspection at the center was completed by the health department 8/15/18 and the center is on city water and sewage system.

Plan of Correction: The director will have the form completed.

Standard #: 22VAC40-185-510-C
Description: Based on review of policies/procedures and medications, the center failed to ensure that there were procedures to ensure the use of outdated medication. Evidence: 1. There was no written procedure in the medication policy for how to prevent the use of outdated medications. 2. Two medications for one child were observed to have been used past the expiration date of the medication. One medication was albuterol sulfate 90mcg that was given to the child eight times in the month of October. This medication's expiration date as noted on the canister by the manufacturer was 4/18. The second medication for the same child was beclomethasone dipropionate 40mcg and had been given to the child 62 times in September, October and November 2018. The medication's expiration date as noted on the canister by the manufacturer was 8/18.

Plan of Correction: The director will return the medications to the parent and add a procedure to the medication policy for prevention of use of outdated medications.

Description: Based on observation and interview, the center failed to ensure that the shelter-in-place locations were posted on the emergency maps. Evidence: 1. The shelter-in-place locations (tornado drill) were not posted on the posted emergency maps.

Plan of Correction: The director will ensure that the shelter-in-place locations are noted on the emergency maps.

Standard #: 22VAC40-185-560-M
Description: Based on observation, the center failed to ensure that staff sat with the children during meal times. Evidence: 1. Four classes were observed during the breakfast meal. No staff were observed sitting with the children during the meal time.

Plan of Correction: The director will remind staff to sit with the children during meals.
