Comments:
An unannounced monitoring inspection was conducted with the director beginning at 10:00am and ending at 12:45pm. There were 42 children in the care of six staff during the inspection. The children's ages ranged between three and four years old. Children were observed participating in free play, circle time, singing, and learning about the letter G and galaxies. Five children's records and five staff records were reviewed and in compliance. Two medications for one child were reviewed and complete. Two staff have Medical Administration Training (MAT) certificates. There were staff on site with first aid, cardiopulmonary resuscitation (CPR) and Daily Health Observation training. The most recent emergency escape drill was conducted on 11/2/2018 and the shelter in place drill was conducted on 10/13/2018. The last fire inspection was completed on 5/4/2018 and the last health inspection was completed on 4/25/2018. Standard #: 22VAC40-185-270-A
Description: Based on observation of the playground not all of the areas and equipment were maintained in a safe condition. Evidence: On the playground there was duct tap holding down a mat on the lower platform of the playground structure with part of the duct tap was unattached and presenting a tripping hazard.

Plan of Correction: Maintenance removed the mat and tap during the inspection.

Standard #: 22VAC40-185-280-A
Description: Based on observation hazardous substances or harmful agents were not stored in a locked place to prevent access by children. Evidence: In the Three year old classroom, 2- 1oz. bottles of gel hand sanitizer were in an unlocked cabinet drawer. In the four year old classroom, ten ice packs containing harmful substances were stored in a bottom drawer of an unlocked cabinet.

Plan of Correction: The hand sanitizer and ice packs will be removed and stored in a locked place.

Standard #: 22VAC40-185-430-C
Description: Based on observation of the playground it was determined that swings had S-hooks with openings greater than the thickness of a penny. Evidence: The S-hooks on the playground swing set had openings greater then a penny presenting an entanglement hazard.

Plan of Correction: Administration will contact county playground department to close all S-Hooks. The swings will be placed all limits until the repairs are made.
