Technical Assistance:
As discussed proof of identity needs to include the date and person who viewed the documentation.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 42 children with 4 staff on site. Children were observed arriving for the program directly after school. The children placed their personal belonging in individual bins and sat at the tables for attendance. Staff strategically placed throughout the program area to ensure proper supervision. The large group was broken into two groups, one group went out to the hall to participate in table top games such as Connect 4 and Scrabble. Magazines were used to make holiday trees. The other group remained in the cafeteria and washed hands prior to snack. Snack today was juice or milk and gold fish. Art kits were available to make a scarecrow. Several children were actively engaged in this activity. All medications were reviewed and authorizations were determined complete. A total of 7 children's records were reviewed and found complete. MAT (medication administration training) training, Daily Health Observation training and first aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office, which includes background checks for staff. This inspection began at 1:50pm and ended at 3:15pm.
