Comments:
The inspector conducted an unannounced mandated inspection from noon until 2:00. There were 33 children in care with staff supervising at all times. Two family workers were also present. Children were observed eating lunch, washing hands after lunch, and preparing for a nap. One medication and corresponding paperwork was reviewed. The violations had been corrected from the last inspection. Five children's records were reviewed and three staff records were reviewed. The area of non-compliance is identified on the violation notice. Please contact your inspector at 276-698-9981 with any questions.

Description: Based on observation and measurement, there was not adequate resilient surfacing under and around equipment. Evidence: 1. The inspector measured the mulch and it was 1-3 inches around the slides and play equipment.

Plan of Correction: Mulch has been ordered and will be spread.
