An unannounced monitoring inspection was conducted beginning at 4:10pm and ending at 6:30pm. There were 28 children in care being supervised by four staff members. Children's ages ranged from five to nine years old. The children were observed, receiving homework help and participating in table games. Snack served today was cinnamon toast cereal and milk. Five children's records were reviewed and in compliance. Four staff records were reviewed. Four medications for two children were reviewed and in compliance. There is currently one staff member on site who is MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted on 11/14/2018. Standard #: 22VAC40-185-540-E Description: Based on observation the center did not have a working battery-operated radio. Evidence: The center did not have batteries for the radio. Plan of Correction: The center ordered batteries on-line during the inspection.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of four staff file, the facility failed to have documentation of a completed central registry finding (CPS check) within 30 days of employment. Evidence: Staff A (date of hire 8/21/2018) did not have documentation of a completed CPS check within 30 days of employment. Plan of Correction: Staff A will fill out a new central registry form and send it out.
