An unannounced monitoring inspection was conducted on 11/14/18 from 9:43am - 12:15pm. During the inspection there were 42 children ages three months old through four years old in care with 13 staff. Children were observed participating in various activities in the classrooms, eating lunch, and during rest period. Records were reviewed for five children and five staff. There is no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-130-B Description: Based on a review of five children's records, it was determined that the licensee did not ensure that the center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #2 (18 months old) contained an immunization record that was dated 12/19/17, which was completed when the child was 7 months old which more than 6 months ago. 2. The record for child #5 (3 years old) contained an immunization record that was dated 8/24/16, which was completed when child #5 15 months old. 3. Staff #6 (Program Director) reviewed the records for child #2 and child #5 and confirmed that updated immunizations were not received every six months for child #2 and child #5. Plan of Correction: The facility responded: The parents of child #2 and child #5 were contacted during the inspection and asked to provide updated immunization records. We will review all current children's records to ensure that immunization records are updated every six motnhs.
Standard #: 22VAC40-185-550-M Description: Based on a review of ten injury reports, it was determined that the licensee did not ensure that the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and contain all of the items required by the Licensing Regulations. Evidence: 1. Two of the ten injury reports did not include the action to prevent further recurrence. 2. Staff #6 (Program Director) reviewed the injury reports and confirmed that two of the injury reports did not cotnain all of teh required information. Plan of Correction: The facility responded: Staff will be re-trained to ensure that all injury reports are completed correctly. Center management will review each report to ensure all areas of the injury reports are completed.
