An unannounced monitoring inspection was conducted from 10:30 am-12:30pm. At the time of the tour there were 53 children in care with 14 staff present. Children were observed during the center's "Thanksgiving" meal and celebration with parents and staff. Infants were observed sleeping in cribs and being fed.
Standard #: 22VAC40-185-140-A Description: Based on a record review the center did not ensure each child had a physical examination under the direction of a physician. Evidence: Child #3 did not have documentation of having a physical examination. Child #3 did not have documentation of a start date in the record. The program director confirmed during the inspection child #3's first date of enrollment was 10/11/16. Plan of Correction: This will be corrected by 11/16/18. Going forward a student will not start unless all paperwork is complete. All current children's files will be reviewed and updated to ensure compliance by 11/30/18. Management will review periodically to ensure compliance.
Standard #: 22VAC40-185-60-A Description: Based on a record review the center did not ensure all required information was kept for each child enrolled. Evidence: Five children's records were reviewed. 1. None of the five records had documentation of their first date of enrollment. 2. Child #1 did not have documentation of phone numbers for 2 required emergency contacts. 3. Child #2 and #3 did not have documentation of the address for 2 required emergency contacts. Plan of Correction: This was corrected on 11/16/18. Going forward a student will not start unless all paperwork is complete. All current children's files will be reviewed and updated to ensure compliance by 11/30/18. Management will review periodically to ensure compliance.
Standard #: 22VAC40-185-280-D Description: Based on observation the center did not ensure cleaning and sanitizing materials were kept separate from food. Evidence: In the infant 2 room there was baby formula stored in the same cabinet shelf as a sanitizing spray and a large container of soap. Plan of Correction: This was corrected immediately after visit on 11/13/18. Going forward our appointed safety captain will not assume the responsibility of doing daily room checks to ensure compliance. Management will inspect periodically.
Standard #: 22VAC40-185-440-H Description: Based on observation the center did not ensure cribs met all requirements. Evidence: In the Infant room there was a child observed sleeping in a crib that had more than 1 inch of space between the mattress and the crib. Plan of Correction: This was corrected on 11/13/2018. Going forward daily classroom inspections by the infant teachers will ensure compliance.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a record review the center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: Staff # 3 (hire date 08/20/18) indicated living outside the state of Virginia within the past five years on the sworn statement of affirmation. Staff #3 did not he have documentation of having a central registry check in the state indicated on the sworn statement. Plan of Correction: This was corrected during visit on 11/13/18. All staff paperwork must be completed on day 1 of hire. All follow-ups will be documented on the copy of the form to ensure compliance. Management will periodically inspect to ensure compliance.
