The licensing inspectors conducted an unannounced monitoring inspection on Wednesday, November 7, 2018 from approximately 2:32 pm to 4:52 pm to determine the center's compliance with licensing standards. Upon the licensing inspector arrival, the children were observed engaged in play and interacting with their peers on the various equipment at the playground area. The children were also observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 54 children and five staff including administrative staff. The licensing inspectors observed the children in care completing a variety of activities to include transition indoors from outdoor play, completing bathroom breaks, entering classrooms, participating in roll call, and preparing for snack time. The children's snack consisted of mixed crackers and water. The licensing inspector completed observations of parent pick up. The facility's playground area, bathrooms areas, and classrooms/materials were inspected. There were five children?s records and three staff's record reviewed during this inspection. The required posting were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. Description: Based on review of children's record and interview with staff, the center did not ensure to obtain required documentation for immunization for each child.

Evidence: During the inspection on November 7, 2018, in Child#1's record (start date 09/28/18) there was no documentation of an immunization record found. Interview with staff confirmed that a copy of immunization record had not been received at this time.

Plan of Correction: Child has appointment on 11/14/18. Parent will be requested to bring in immunization documentation immediately.

Description: Based on review of children's record and interview with staff, the center did not ensure a physical examination for each child.

Evidence: During the inspection on November 7, 2018, in Child#1's record (start date: 9/28/18) a copy of physical examination was not found. Interview with staff revealed the physical was not available during the inspection.

Plan of Correction: Physical examination is schedule for 11/14/18

Standard #: 22VAC40-185-270-A
Description: Based on observations at the facility, the center did not ensure all areas were maintained in a clean, safe and operable condition.

Evidence: During the inspection on November 07, 2018, the following inside and outside areas were observed:
1. There were multiple areas of visible rusting on the swing chains and poles on the playground area.

2. The toilet bolt cap was missing in the bathroom exposing a rusted screw.

Plan of Correction: The corrections to all areas will be made by 11/14/2018.

Standard #: 22VAC40-185-350-A
Description: Based on observations and interview with staff, the center did not ensure the required staff -to children ratios were followed.

Evidence: During the inspection on November 7, 2018, there were observations of one staff and 22 school-aged children on the playground are at time of arrival and during a formal observation of children in care. Interview with staff confirmed the children head count.

Plan of Correction: The program will ensure to have proper staff to meet the staff to children ratios.
