Technical Assistance:
Consultation was given on acceptable forms of proof of identity and a lead teacher supervising no more than 2 assistants.

An unannounced monitoring visit was conducted today from 11:00-1:00. All rooms were in ratio with direct care staff. Observed children transitioning from lunch, diapering, reading and preparation for rest time and infant care. Lunch served today was beans and rice, salad, tortilla and fruit. Infant bottles and over the counter creams were properly labeled. Reviewed a sample of children and staff records, teacher background checks, trainings, medication and emergency preparation. Description: Based on record review, 1 of 6 children did not have an immunization record prior to attendance. Evidence: Child #1 started attending the center 6/25/2018 and no immunization record is available in the child's record.

Plan of Correction: Center management will request immunization record from the parent.

Standard #: 22VAC40-185-130-B
Description: Based on record review, 2 of 6 children did not have an update of additional immunizations once every six months for childrn under the age of two years. Evidence: 1. Child #2's date of birth is 3/2017 and the last immunization update was 5/2017. 2. Child #3's date of birth is 9/2017 and the last immunization update was 5/2017

Description: Based on record review, 1 of 6 children's records is incomplete. Evidence: Child #2 is missing proof of identity as a social security number does not meet one of the three acceptable means of verification.

Description: Based on medication review, a written authorization is missing the parent signature. Evidence: Child #4's medication authorization is missing the written signature of the parent.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, 2 of 4 long term medications are missing current authorization signatures. Evidence: Child #5's medication authorization signatures from the parent and physician expired September 2018.

Plan of Correction: Updated authorization requested from parent.

Standard #: 22VAC40-185-550-M
Description: Based on review of injury records, 5 of 10 records are incomplete and not containing all required information. Evidence: 1 record is missing future action to prevent reoccurrence, 1 is missing the date the parent was notified, 1 was missing the method the parent was notified and 2 were missing the time the parent was notified.

Plan of Correction: Staff retraining. Forward written procedures to all lead teachers. Review, sign off with staff.
