Comments:
An unannounced monitoring inspection was conducted from 10:45am-2:00pm. At the time of entrance, 53 children were in care of 9 staff members present. Children were observed engaged in gross motor activities, play with manipulatives, storytime, handwashing and lunch. A selection of 5 staff and 6 children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on records reviewed, 1 out of 6 children's records did not include documentation of a physical examination within 30 days of enrollment. Evidence: The record for Child #2 [date of enrollment: 8/27/2018] was missing a physical exam.

Plan of Correction: We will request the information from the parent.

Standard #: 22VAC40-185-160-C
Description: Based on records reviewed, 1 out of 5 staff records did not include results of subsequent tuberculosis screenings every two years from the date of the initial screening. Evidence: The most recent tuberculosis screening on file for Staff #5 was obtained 9/2/2016.

Plan of Correction: I will make sure the staff member obtains documentation of a new screening.

Standard #: 22VAC40-185-240-D-1
Description: Based on records reviewed, documentation of staff completion of medication administration training was not available. Evidence: Documentation of training to administer an emergency medication observed on site was not available during the time of the inspection.

Plan of Correction: Inspector will provide training resource via email; We will complete and file documentation of the training.

Standard #: 22VAC40-185-270-A
Description: Based on observations, areas inside the center were not maintained in clean and safe conditions. Evidence: 1. An area of chipped and peeling paint was observed on a wall in the 2-4 year old classroom. 2. Stains were observed on a soiled diaper changing pad in the toddler bathroom.

Plan of Correction: We will have the area repainted.

Standard #: 22VAC40-185-430-B
Description: Based on observations, manipulative equipment was not age and stage appropriate for children. Evidence: A basket of small counting bears and threading cubes were observed in a toddler room - posing a choking hazard.

Plan of Correction: The items will be removed.

Standard #: 22VAC40-191-60-C-2
Description: Based on records reviewed, 1 out of 6 staff records did not include results of a central registry check within 30days of employment. Evidence: There were no central registry results available at the time of inspection for Staff #1 [8/20/2018].

Plan of Correction: The results were re-mailed and we are awaiting the results.
