An unannounced monitoring inspection was conducted today from 10:00am-12:00pm. There were 31 children directly supervised by 54 staff. The physical plant, 5 staff records, 6 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing with cars and trucks and building with blocks. There was a supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not ensure within one month after attendance each child had a physical examination by or under the direction of a physician. Evidence: Child 1 and 2 did not have documentation of a physical examination of a physical. (Child 1, DOE 12/14/17; Child 2 DOE 12/14/17.) Plan of Correction: We will each out to the parents to get the physicals.
Standard #: 22VAC40-185-160-C Description: Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each staff member every two years. Evidence: 1) Staff 1 has a tuberculosis tests conducted on 10/19/16. 2) Staff 2 has a tuberculosis tests conducted on 7/7/16. 3) Staff 3 has a tuberculosis tests conducted on 6/6/16. Plan of Correction: All the staff will get updated tuberculosis tests.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not maintain all equipment in the center in a safe condition. Evidence: In the upstairs classroom, there is a play kitchen missing a piece of equipment leaving a hole which presents a potential finger entrapment for the children. Plan of Correction: We will fix the hole so it is not exposed.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not ensure each staff member had a central registry finding within 30 days of employment. Evidence: Staff 4 does not have a central registry finding in their record. (Staff 4, DOH 1/2/15) Plan of Correction: We sent it previously but did not receive it back on time. We will do a new one.
