Comments:
An unannounced renewal inspection was conducted from 10:25am-1:30pm. 73 children were in care with 12 staff members present. Children were observed engaged in group activities including but not limited to playing with toys, coloring, doing worksheets, reading books and playing bingo. 5 staff and 5 children records, medications, emergency drill records, emergency preparedness plans and supplies were reviewed. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas inside and outside of the center were not maintained in a safe and operable condition. Evidence: 1. The wooden fence for the toddler playground had planks that were bowing, causing large gap making it an entrapment hazard for the children. 2. There was peeling paint found in the Pre-K classroom where the cots are stored.

Plan of Correction: A work order will be submitted for both things that need to be fixed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not stored in a locked cabinet making them accessible to the children. Evidence: In the Discovery preschool classroom, diaper rash creams were stored in a cabinet under the diaper changing table. One of the two door handles was broken and safety lock could not be used, leaving the cabinet unlocked and making the topical ointments accessible to the children.

Plan of Correction: A work order will be placed for the cabinet and the diaper rash creams will be placed in a different cabinet.

Standard #: 22VAC40-185-510-G
Description: Based on observation, the center had medication on site that was expired. Evidence: There was an emergency medication on site for a child with a diagnosed food allergy. The medication expired in October 2018.

Plan of Correction: The medication will be returned to the parent and new medication will be requested.
