63.2 Facilities & Programs.

An unannounced monitoring inspection was conducted on November 28, 2018, between the hours of approximately 10:30 a.m. and 3:45 p.m. Fifty-five (55) children were enrolled and forty-two (42) children were in the care of eleven (11) staff members during the on site inspection. One additional staff member was present and assisted, as needed. Five children's records and seven staff records were reviewed and/or updated. One child's medications, three children's topical ointments, accompanying authorization forms and one child's medication administration form were reviewed. Safety equipment (first aid kits, flashlights and radios), posted information (license, licensing inspection documentation, emergency evacuation diagram/plan, emergency telephone numbers, and classroom schedules) and available documents (emergency evacuation drill & shelter-in-place records, attendance logs, infant daily records, injury report forms, insurance verification and one local agency report) were observed/reviewed. The daily schedules accurately reflected the activities provided. Lunches and snacks were provided by the children's parents. Activities observed on this date included: indoor play/art - making holiday wreaths, coloring and stories; infant care; diapering, restroom and handwashing procedures; lunch; naptime and pick-up/departure of children. Staff/child interactions were observed to be positive, supportive and age appropriate. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Substantial compliance with the licensing standards continues to be maintained; one exception to standards compliance was noted on the accompanying violation notice. Description: Based on observation of diapering procedures and discussion with staff on November 28, 2018, the program staff failed to ensure the immediate safety of a child during diapering. Evidence: 1. Observation of diapering procedures, on this date, determined that staff member #2 stepped away from a child on the changing table to dispose of a soiled diaper. 2. Staff #2 confirmed that although she had strapped the child onto the changing table, she stepped away from the changing table to dispose of a soiled diaper.

Plan of Correction: The staff member (a three/four year-year-old teacher) is not normally in that classroom. Staff members will be reminded to follow the posted diapering procedures.
