An unannounced monitoring inspection was completed on 11/19/2018. There were 36 children in care with ten staff supervising the children. The inspector arrived at 7:45 A.M. and departed the program at 12:00 PM. Five children's and five staff's and three board officer's record were reviewed during the inspection. Children were observed: arrival, morning snack, outside time, free play, diapering, and morning snack. Discussion with director about: location of directors personal items, reducing age capacity, measurement of the school age room with out the half wall to add one additional child, and mulch.
Standard #: 22VAC40-185-130-B Description: Based on review of five children's records, the center failed to ensure that all immunization records were updated every six months for children under the age of 2 years. Evidence: 1. Child # 1 most recent immunization record was completed on 1/13/2017. The child's DOB is 3/14/2016. Plan of Correction: The director stated the child will be going to the doctor soon and will have the parent to bring in an updated record and will monitor all forms.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to ensure that all areas and equipment of the center was maintained in a safe, clean and operable condition. Evidence: 1. One of the three high chairs in use had a crack in the tray that posed a pinch hazard and a sharp cutting hazard to the children. The high chair was observed twice in use during the inspection. 2. In the toddler room, the edge of the change table had a sharp portion that posed a hazard to the children in care. Plan of Correction: The high chair was discarded during the inspection. The edge of the changing table will be sanded down to a smooth edge. All areas and equipment will be monitored.
Standard #: 22VAC40-185-350-C Description: Based on observation, the center failed to ensure that all ratios were met according to the youngest child in the in the grouping. Evidence: 1. The preschool room was observed at 7:52 there was 11 children 2 1/2 year of age to four years of age with one staff person. There was an additional person in the kitchen preparing breakfast. That staff person was placed in the room at 7:56 to meet the ratio until the second staff person arrived. The required ratio for this mixed age grouping is eight children to one staff person. Plan of Correction: The director will rearrange schedules to accommodate the incoming children and will monitor and rearrange as needed.
